However, not all obese individuals experience metabolic abnormalities. A recent study conducted in Iran sought to investigate the association between dietary insulin index (DII) and dietary insulin load (DIL) with metabolic health status and the serum levels of brain-derived neurotrophic factor (BDNF) and adropin among the Iranian adult population. The findings shed light on the impact of diet on metabolic health and offer valuable insights into potential interventions.<\/p>\n<h3>The Role of Diet in Metabolic Health<\/h3>\n<p>Obesity is often associated with insulin resistance, hypertension, and hypertriglyceridemia. However, a significant percentage of individuals with obesity do not experience these metabolic abnormalities. Factors such as cardio-respiratory fitness, lifestyle, chronic stress, and adipose tissue function contribute to metabolic health status. Insulin resistance, which leads to chronic inflammation, is also linked to metabolic health. Diets that elevate blood sugar levels increase the risk of insulin resistance. The DII measures postprandial insulin secretion after consuming common foods, while the DIL provides information about the insulin index of each food and its energy content.<\/p>\n<h3>The Role of BDNF and Adropin<\/h3>\n<p>BDNF is a neurotrophic growth factor that plays a crucial role in reducing the risk of type 2 diabetes, cardiovascular disease, obesity, hyperglycemia, metabolic syndrome, and dyslipidemia. Adropin, a short peptide hormone, is expressed in various organs and has been associated with metabolic disorders. Interestingly, both BDNF and adropin are influenced by dietary components.<\/p>\n<h3>Previous Studies and Knowledge Gap<\/h3>\n<p>Previous studies have established a link between DII and DIL with metabolic disorders such as type 2 diabetes and obesity. Higher DIL has been associated with an increased risk of insulin resistance. However, no population-based studies have been conducted to investigate the association between DII and DIL with adropin and serum BDNF in relation to metabolic health among Iranian adults.<\/p>\n<h3>The Study Design and Findings<\/h3>\n<p>The cross-sectional study included 527 adults from Isfahan, Iran, who were categorized into metabolically healthy (MH) and metabolically unhealthy (MU) groups. The study found that adherence to a diet with a high DII increased the odds of being in the MU group. However, no significant association was observed between DIL and metabolic health status. A higher DII was linked to increased blood pressure, while moderate DIL was associated with hypertriglyceridemia. Surprisingly, no significant association was found between DII and DIL with adropin and serum BDNF levels.<\/p>\n<h3>Implications and Recommendations<\/h3>\n<p>The findings of this study suggest that individuals, regardless of weight status, should reduce their consumption of foods with a high DII to improve diet quality and decrease the burden of metabolic diseases. Foods such as refined grains, sugar, potatoes, and desserts, which have a significant impact on insulin response, should be avoided or limited. These dietary modifications have the potential to improve metabolic health and enhance overall quality of life.<\/p>\n<h3>Limitations and Future Directions<\/h3>\n<p>The study has some limitations, including the use of self-administered food frequency questionnaires for dietary assessment, which may introduce misclassifications and biases. The presence of unmeasured confounders and the inability to establish causality due to the cross-sectional design are also limitations. Future research should explore the long-term effects of dietary insulin index and load on metabolic health and consider other factors that may influence the association.<\/p>\n<h3>Conclusion:<\/h3>\n<p>The study conducted among Iranian adults highlights the association between dietary insulin index and metabolic health status, shedding light on the impact of diet on metabolic health. Adherence to a diet with a high DII was found to increase the risk of metabolic abnormalities, while no significant association was observed with dietary insulin load. These findings emphasize the importance of dietary interventions in improving metabolic health and reducing the burden of metabolic diseases.
